Open Partial Nephrectomy: Results from an 8 Year Study

Archives of Clinical Nephrology. Research Article Abstract  Introduction: Nephron sparing surgery is an effective treatment for RCC. Oncological outcome is equivalent to radical nephrectomy, with the added functional advantage. It is underutilized, especially in tumours in anatomically challenging positions like central and hilar positions. Methods: We analyzed the hospital case notes and electronic records of 90 consecutive cases of partial nephrectomies, done at our hospital over the last 8 years. We collected data on the tumours number, size, position (upper zone, central/hilar and lower zone), histology, margin status, operative time, ischemic time, peri-operative complications, length of stay and recurrences so far. We analysed these data according to tumour site (hilar/central vs. polar).   http://www.peertechz.com/Clinical-Nephrology/pdf/ACN-1-106.pdf

Oxidative Stress as “Mother” of Many Human Diseases at Strong Clinical Impact

Journal of Cardiovascular Medicine and Cardiology Review Article Abstract  Oxidative stress, characterized by the production in excess of free radicals, is the main aspect of all living systems which use oxygen to convert biochemical energy coming from nutrients into adenosine triphosphate. In turn free radicals, also called reactive oxygen species , induce oxidative damage to some cellular macromolecules, as lipids, proteins, and DNA .Increased reactive oxygen species serum concentration has been implicated in the pathogenesis of some, common human diseases, included both healthy and diseased ageing. The most frequent pathologies involved are: atherosclerosis, cancer, Alzheimer’s and Parkinson’s Diseases and chronic obstructive pulmonary disease. Together with these, other, less frequent diseases can be interested, as chronic fatigue syndrome, lateral amyotrophic sclerosis and skin diseases. Therefore oxidative stress, that is an imbalance of an essential biochemical reaction phys

Advanced Shape Memory Technology for Biomedical Engineering

Peertechz Journal of Biomedical Engineering Editorial   The ability to recover to the original shape only at the presence of the right stimulus is traditionally known as the shape memory effect (SME) [1]. The materials with such a capability are technically termed shape memory materials (SMMs) [2]. Typical SMMs include shape memory alloy (SMA) and shape memory polymer (SMP, including hydrogel), while typical stimuli are temperature (thermo-responsive, both heating or cooling), chemical (chemo-responsive, including water), light (photo-responsive) and magnetic (magneto-responsive) [3]. Although SMMs have been mostly used for actuators in the past, they have been proposed for new types of sensors as well, but not so successful till today [4-7] http://www.peertechz.com/Biomedical-Engineering/pdf/PJBE-1-105.pdf

Management of Hepatitis C Infection with Direct Action Antiviral Drugs (DAA)**

Archives of Hepatitis Research Review Article Abstract  Several new direct action antiviral drugs (DAA) against the HCV virus (HCV) are approved and marketed. The combination of DAA with pegylated interferon and ribavirin (PR) is only recommended to patients with tolerance to interferon (IFN). IFN-free treatments are now considered elective drugs for patients with chronic HCV. More than 90% of patients infected with HCV genotype 1 or 4 (fewer with other genotypes) show sustained virological response, when treated with sofosbuvir combined with simeprevir, daclatasvir or ledipasvir, or by the combination of paritaprevir with ritonavir, ombitasvir and dasabuvir, with or without ribavirin. http://www.peertechz.com/Hepatitis-Research/pdf/AHR-1-103.pdf

Endothelial Progenitor Cells in Breast Cancer

International Journal of Immunotherapy and Cancer Research Editorial Abstract  Recent evidences demonstrated the importance of bone marrow derived Endothelial Progenitor Cells (EPC), in the contribution to postnatal physiological and pathological neovascularization, and in tumor growth and angiogenesis. These cells are recruited undifferentiated, in response to systemic or chemoactractive signals, such as Vascular Endothelial Growth Factor (VEGF), they lodge in the growing or lesioned tissue and differentiate into endothelial cells in response to local stimuli and cellcell interactions. The extent and the significance of the EPCs contribution for the growing of most tumors, including those of the breast, are still not fully defined. We analyzed the peripheral blood of breast cancer patients and found that they have circulating EPCs. We also found an association between expression of AC133+Kdr+ and VEGF plasma levels in these patients. Strategies to impair the mobilization and incor

Hemodialysis with Polymethylmethacrylate Restores the Response to Hepatitis B Vaccination in Chronic Dialysis Patients: Hypothesized Mechanism of Action

Archives of Renal Diseases and Management Research Article Abstract  Patients undergoing hemodialysis often present with a reduced response to anti-hepatitis B virus (anti-HBV) vaccination. The soluble form of CD40 (sCD40) is elevated in hemodialysis patients and this has been shown to correlate with lack of response to anti-HBV vaccination. Due to its high molecular weight, conventional dialyzers cannot clear sCD40. Previous studies have demonstrated, that dialysis membranes in polymethylmethacrylate (PMMA) can reduce the levels of sCD40. We have studied the effect of dialysis with PMMA membranes in patients who were non-responders to anti-HBV vaccination after a complete cycle of vaccinations. Interestingly, we found that significantly more patients in the PMMA group were able to mount a response to vaccination, compared to the control group (P = 0.04).  http://www.peertechz.com/Renal-Diseases-Management/pdf/ARDM-2-106.pdf

The Correlation Between Arterial Hypertension and Endothelial Function

Archives of Clinical Hypertension Editorial Arterial hypertension is defined by a stable increase in systemic arterial blood pressure (BP) values, i.e. systolic value of 140 mmHg or more and/or diastolic one of 90 mmHg or more. Its prevalence is about 30–45% of the general population; representing a well-known cardiovascular (CV) risk factor [1]. In addition to BP values, the assessment of target organ damage has a pivotal role in stratification of total CV risk of patients. Current guidelines for the management of arterial hypertension suggest several tools for evaluating hypertensionrelated asymptomatic organ damage, such as electrocardiography, echocardiography, vascular ultrasound examination (carotid wall thickening or plaque, carotid-femoral pulse wave velocity, anklebrachial index), and estimated glomerular filtration rate and micro albuminuria for study of renal function   http://www.peertechz.com/Clinical-Hypertension/pdf/ACH-1-104.pdf

Anterior Tarsal Tunnel Syndrome with Presence of Accessory Deep Peroneal Nerve: Case Report

Journal of Neurology, Neurological Science and Disorders Case Report Abstract Entrapment neuropathy of the deep peroneal nerve, also recognized as anterior tibial nerve, typically occurs at the anterior ankle and dorsal foot. Compression of this nerve, which anatomically is inferior to the extensor retinaculum, is commonly referred to as anterior tarsal tunnel syndrome. This syndrome is rare and remains poorly diagnosed among clinical problems. A 25-year-old woman was referred to our outpatient clinic for a second opinion. She presented with a vague pain over the dorsomedial aspect of the foot which occasionally radiated into the first intertarsal space. Electromyography revealed moderate prolongation of distal latency and presence of accessory deep peroneal nerve (ADPN) with partially innervated extensor digitorum brevis. Our diagnosis was anterior tarsal tunnel syndrome, and surgical decompression of the anterior tarsal tunnel was performed. At the follow up three months
